What is a Care Manager?
A Care Manager explores what the client needs are, what their priorities are as well as economic abilities and then develops and implements a care plan based on who in the support system can best accomplish each task. A Care Manager often expands a senior's support system and helps to arrange for and monitor community services using their unique community connections, experience and expertise to promote independence, health and quality of life for their clients.
How do I get started?
The process begins by first conducting an in-home needs assessment. Through this we conduct evaluations to establish a personalized care plan and identify any obstacles to living independently and safely.
After this we screen, arrange and monitor in home help and medical equipment and delivery and installation of durable medical equipment. Additionally, we review financial, legal, and medical issues and provide referrals where necessary.
What assistance is provided?
We accompany clients to appointments with physicians, attorneys, accountants, etc and facilitate communication between physicians and other healthcare providers as well as liaising with long-distance caregivers . We also provide support for day-to-day activities which include:

Transportation to medical appointments, errands and social activities

Support for responsibilities of home ownership and maintenance

Assistance with bill paying and monitoring for potential fraud
What about special circumstances?
Our staff is ready to respond to even the most unexpected and specialized circumstances. In addition to providing crisis intervention services and specialized problem solving, we can arrange home safety evaluations and provide assistance with home modifications. We also provide coaching to family and caregivers, and can even help with coordination of downsizing, or a move to a Senior Housing Community (local or long-distance).
Can you help with Senior Housing Communities?
In addition to providing an objective critique of residential care facilities, we can also arrange placement in a Senior Care Community (retirement living, independent living, assisted living, personal care home, or skilled nursing facility). We also provide ongoing, onsite monitoring of care communities and their quality of care. And if the relocation isn't local we provide fly-along or driving companionship for relocations to and from Atlanta.
